Select the specific reason: name change, address change, damage, loss, or exhausted pages.
Fill the form and attach the reason-specific supporting document.
Pay the applicable fee and book a PSK appointment.
Visit the PSK with the old/damaged passport (or FIR/affidavit if lost) and other originals.
Undergo verification, which may include a police enquiry depending on the reason.
Track the application and collect the newly printed passport.
