Ayushman Bharat (PM-JAY) — Apply & Download Card

Ayushman Bharat PM-JAY (Pradhan Mantri Jan Arogya Yojana) gives eligible families cashless health cover of up to ₹5 lakh per year at empanelled government and private hospitals. Eligibility is based on the SECC 2011 database plus specific occupational categories — it isn't open enrolment. Since 2024, all senior citizens aged 70+ are also covered under the scheme regardless of income, issued a separate Ayushman Vay Vandana Card. You can check eligibility, download your e-card, and find empanelled hospitals entirely through the official PM-JAY portal or app — never through a third-party 'card maker'.

Required documents

  • Aadhaar number (for identity verification and e-KYC)
  • Ration card or family ID, if asked to cross-check household details
  • Mobile number for OTP verification

Eligibility

  • • Rural and urban families identified as eligible under the SECC 2011 socio-economic database
  • • Households already covered under RSBY (Rashtriya Swasthya Bima Yojana) in eligible states
  • • All senior citizens aged 70 and above, regardless of income (Ayushman Vay Vandana Card, added 2024)
  • • State-specific expanded categories in states that have extended PM-JAY (varies by state — check the portal or your state's own scheme page)

Step-by-step guide

  1. 1

Go to the official beneficiary portal at mera.pmjay.gov.in, or use the Ayushman App / PM-JAY mobile app.

  2. 2

Use 'Am I Eligible' and enter your mobile number, state, and either your ration card, RSBY URN, or Aadhaar to search the eligibility database.

  3. 3

If eligible, complete e-KYC via Aadhaar OTP or fingerprint at a CSC (Common Service Centre).

  4. 4

Download your Ayushman e-card directly from the portal, or get a printed card at a CSC / empanelled hospital's Ayushman Mitra desk.

  5. 5

For treatment, visit any empanelled hospital and show your Ayushman card and Aadhaar — the Ayushman Mitra there verifies and processes cashless admission.

Official fees

Checking eligibility and downloading the e-card is free. Some CSCs charge a small, government-notified printing/service fee for a physical card — never pay a large sum to anyone claiming to 'issue' or 'guarantee' a card.

Frequently asked questions

Use 'Am I Eligible' on mera.pmjay.gov.in or the PM-JAY app with your mobile number and state — it checks the official SECC 2011 database directly.

Common mistakes to avoid

  • • Paying anyone who claims they can 'guarantee' or fast-track a card outside the official portal/CSC
  • • Not verifying a hospital is currently empanelled before assuming treatment will be cashless
  • • Assuming the scheme is open enrolment — eligibility depends on the SECC database or the 70+ category, not on applying alone
  • • Losing the Ayushman card and not realising it can simply be re-downloaded from the portal for free
