Go to the official beneficiary portal at mera.pmjay.gov.in, or use the Ayushman App / PM-JAY mobile app.
Use 'Am I Eligible' and enter your mobile number, state, and either your ration card, RSBY URN, or Aadhaar to search the eligibility database.
If eligible, complete e-KYC via Aadhaar OTP or fingerprint at a CSC (Common Service Centre).
Download your Ayushman e-card directly from the portal, or get a printed card at a CSC / empanelled hospital's Ayushman Mitra desk.
For treatment, visit any empanelled hospital and show your Ayushman card and Aadhaar — the Ayushman Mitra there verifies and processes cashless admission.
Official fees
Checking eligibility and downloading the e-card is free. Some CSCs charge a small, government-notified printing/service fee for a physical card — never pay a large sum to anyone claiming to 'issue' or 'guarantee' a card.
