Log in to mera.pmjay.gov.in with your registered mobile number and OTP, or visit the nearest CSC / empanelled hospital's Ayushman Mitra desk.
Open your beneficiary profile and select the correction/update option.
Choose the field to correct (name, DOB, gender, address, or family member details) and enter the corrected value as it appears on your Aadhaar.
Complete Aadhaar-based e-KYC (OTP or fingerprint) to verify the correction.
Once approved, download the updated e-card from the portal, or collect a reprinted physical card at the CSC.
Official fees
Correction requests through the portal are free. A CSC may charge a small, government-notified fee only for physically reprinting a card.
